There is nothing as absolute as death. There has not been one; there will not be.

Proper education.

Talk to your care circle and put it in writing.

It is no news that the legal documents are what is mostly tenable when the time comes. It is still good to have a back up in writings to caregivers and family members about how much you would prefer a specific type of treatment.

You could also communicate verbally with loved ones and trust that your wishes would be respected.

Find tools to help an elder reflect on old age.

Anything you can lay your hands on that will serve as a means of reflection. Having to think about one's death may not be palatable, but with sites like PREPARE, you can achieve it with ease.

This has to be someone outspoken and ready to be there at hospital meetings and in case of an emergency is not incapacitated about decisions.

Update the advance directive every year. 

What you felt was okay last year may differ from your feeling this year.

Check the medical record; this is to ensure that the preventive check reflects what has been updated over time.
