**Step - 1.** Go to "File"

**Step - 2.** Select "Options"

**Step - 3** (1). Go to "**Quick Access Toolbar**" ---> (2). Then select "**Choose command form**" and select "Commands Not in the Ribbon" in the dropdown ----> (3). Then Select "**Form...**" in the index

**Step - 4** And Add to it in right side index table ---> Create a new group ----> then click on "**Add>>**" button

**Step - 5.** Create Quick Access Toolbar for "Form..."

**Step - 7.** Create columns with their Names. (Entry fields Name)

And then after Select those Names and then click this icon(given below).
